This publication supports the development of community-campus partnerships that promote the achievement of the Healthy People 2010 Objectives.  The Toolkit contains three pieces:  a Healthy People 2010 Assessment Tool, Declaration and Commitment Forms and a short resource list.  The assessment tool is a helpful overview for evaluating organizational readiness to promote the Healthy People 2010 objectives.  The assessment tool would likely be used by faculty and deans, although the questions could be adapted to any organization involved in providing health care.  The commitment forms are a symbolic gesture indicating willingness to change.
